This voucher is non-refundable.
This voucher can be transferred to the others. Please present the voucher with ID card before check-up at Chemotherapy and Hematology Center, 12th floor, Vichaiyut Medical Center.
Recommended for patient aged 18 years old or above and the patient is required to sign the consent form before screening.
The package is inclusive of doctor fee, service fee and OPD patient service fee
This package is for OPD patient only, cannot be used with other privileges and discounts for shareholders, contract companies, and insurance companies
The package fee will be charged in full amount even though the patient chooses not to take any check-up item and the check-up list cannot be changed
If there is any other treatment apart from the package, the patient will have to pay for any actual expense occurred
The patient will get 10% discount for laboratory and X-ray services only for additional health checkup (radiologist fee excluded). For additional test for treatment is not included in this condition
If the patient has additional checkup, treatment, and medication for treatment, the expense will be charged apart from the package price and paid by the patient in actual cost
This package cannot be exchanged to cash and it can be used only one time, cannot be repeatedly used.
