Physical examination by doctor
Medical certificate (5 diseases) for job application, furthering study application, and driving license extension
The package is inclusive of doctor fee, service fee, and OPD patient service fee. Additional doctor fee may occur should there is any other treatment apart from that listed in the package.
This package is for OPD patient only, cannot be used with other privileges and discounts for shareholders, contract companies, and insurance companies.
The package fee will be charged in full amount even though the patient chooses not to take any check-up item and the check-up list cannot be changed.
The patient will get 10% discount for laboratory and X-ray services only for additional health checkup (radiologist fee excluded). For additional test for treatment is not included in this condition.
If the patient has additional checkup, treatment, and medication for treatment, the expense will be charged apart from the package price and paid by the patient in actual cost
This price is for Thai citizen and the foreigner living permanently in Thailand only.
The package can be purchased from 30 December 2023 – 31 December 2024.
